Whether SC/ST/OBC, Educational Qualifications, Experience (if any), Registration no., duly affixed recent passport size photograph, attested photocopies of testimonials along with a Demand Draft of Rs. 100/- (Rs. 25/- in case of SC/ST) in favour of “ESI Fund Account No. 1” and payable at “Indore” should reach the office of Medical Superintendent, ESIC Model Hospital, Nanda Nagar, Indore – 452011 (M.P.) by 30/10/10 up to 1.00 pm. Envelope containing the application must be superscribed with “Application for the post of Senior Resident”. Women candidates are exempted from fees.
