Application Form : Prescribed OMR application forms and details can be purchased from all Main/ Head Post Offices in all the districts (total 168 ) of UP from 20/07/2011 till last date i.e. 19/08/2011 on payment of Rs.50/-


How to Apply : Application forms complete in all respect should be submitted in the same post offices from where the form was purchased till 5.00 pm 19/08/2011.
